How to Prepare
After scheduling your first exam visit at one of the Eastern Virginia Orthodontics locations, we ask that you fill out our New Patient Forms before coming to the office:
If you are unable to complete the forms prior to your visit, please arrive 10-15 minutes early to fill them out in the office.
What to Bring
Please come prepared to share the following information during your first visit:
- Date of Birth
- Employer
- Insurance Company Name and Group Number
- Name of Subscriber
- Subscriber’s SSN or Policy ID Number

What to Expect
Each patient’s first visit at one of the Eastern Virginia Orthodontics offices is complementary and should last about 45 minutes to an 1 hour. The visit will consist of:
- A Consultation Exam
- Explanation of our Practice
- Detailed Overview of our Services & Treatments
Next Steps
After your initial exam, you will sit down with your doctor to discuss a course of treatment. Typically, we will schedule your next appointment to take images of your mouth, impressions of your teeth and x-rays. This will ensure that Eastern Virginia Orthodontics doctor is able to create a precise and personalized treatment plan to give you the straight smile you came for!
Around 2-4 weeks later, we’ll ask you to come in for a follow-up visit to discuss your personalized treatment plan. We want you to leave our office with an excellent understanding of what your treatment will consist of. During this follow-up treatment, you can expect to learn:
- If intervention is necessary
- If you will need teeth extracted prior to the beginning of your treatment plan
- Estimated duration of your treatment
- The cost and available payment options
